I grant permission and by checking this box I acknowledge that I have read and approve of the foregoing statements:
- I give my permission for my child to participate in Vacation Bible School taking place at Calvary Chapel Church. I understand that the safety of all participants in the activities at the VBS will be the most important concern of the leaders. I understand that there is always a risk of injury, illness, loss and possible consequent expense for medical, diagnostic, and curative treatments.
- In the event of illness or accident, having parental responsibility for the above named child, I give permission for the first aid to be administered.
- If I cannot be contacted and my child should require emergency hospital treatment, I authorize an adult leader to sign on my behalf any written form of consent required by the hospital. However, I understand that every effort will be made to contact me as soon as possible. I do hereby completely release Calvary Chapel El Centro from any responsibility or liability that might arise on account of loss, injury, or consequent expense.
